Mohs micrographic surgery (MMS) has a 5-year cure rate of 99%, 94.4%, 92% to 99%, and 90% for primary basal cell carcinoma (BCC), recurrent BCC, primary squamous cell carcinoma (SCC), and recurrent SCC, respectively.[1][2][3] The utility of MMS extends beyond nonmelanoma skin cancer, including treating rare tumors.[4] The high cure rate consistently reported in MMS relies on the integrity and reproducibility of the prepared histology slides to be accurately interpreted by the Mohs surgeon. Aspects of MMS slide preparation depend on the surgeon and histology technicians. Regardless of practice preferences, the entire care team needs to recognize the technical problems that may impact slide quality. In addition to being competent in obtaining, processing, and staining tissue, clinicians should be aware of common pitfalls encountered at each step and the strategies that should be implemented to reduce potential errors.

Computer-implemented methods and systems are provided for interpreting a whole-slide image of a tissue specimen. Such a method includes detecting biological entities in a region of interest independently at each magnification level in a whole-slide image. An entity graph is generated for each magnification level, the entity graph comprising nodes, representing respective biological entities detected at that magnification level, interconnected by edges representing interactions between entities. The method also includes, for each region of interest, generating a hierarchical graph, defining a hierarchy of the entity graphs for that region, in which nodes of different entity graphs are interconnected by hierarchical edges representing hierarchical relations between nodes of the entity graphs. The method further comprises supplying the hierarchical graph for the region to an AI system to produce result data corresponding to a medical evaluation of the tissue specimen represented thereby, and outputting the result data for the tissue specimen.

This chapter provides an overview of diagnosis in health care, including the committee's conceptual model of the diagnostic process and a review of clinical reasoning. Diagnosis has important implications for patient care, research, and policy. Diagnosis has been described as both a process and a classification scheme, or a “pre-existing set of categories agreed upon by the medical profession to designate a specific condition” (Jutel, 2009).1 When a diagnosis is accurate and made in a timely manner, a patient has the best opportunity for a positive health outcome because clinical decision making will be tailored to a correct understanding of the patient's health problem (Holmboe and Durning, 2014). In addition, public policy decisions are often influenced by diagnostic information, such as setting payment policies, resource allocation decisions, and research priorities (Jutel, 2009; Rosenberg, 2002; WHO, 2012).
